Patient Advocate

• Serve as the primary point of contact for assigned Medicare patients
• Build trusting relationships with patients
• Support patients in understanding care plans and available resources
• Conduct assessments to understand patient needs
• Perform risk assessments related to social determinants of health
• Develop individualized action plans for patients
• Coordinate appointment scheduling and follow-ups
• Identify and refer to community-based resources
• Document patient interactions and outcomes
• Communicate regularly with supervising clinicians

• 3+ years of experience in patient advocacy, care coordination, or related fields
• Strong understanding of the U.S. healthcare system, especially Medicare
• Experience working with older adults or medically complex populations
• Excellent verbal, written, and interpersonal communication skills
• Comfortable using EHRs, CRMs, and patient messaging tools
• Ability to work independently in a remote environment
• Demonstrated competencies in patient communication, relationship-building, and advocacy

Pay range: $22p/h
